Caesarean section
This is the abdominal delivery of the baby by laparotomy and section of the uterus after 28 weeks of pregnancy. Indication of caesarean section.
1> post-caesarean pregnancy (28%)
2> Failure to progression labour.
3> Malpresentation (breech. transverse) (10%)
4> Antepartum haemorrhage (7%)
5> Fetal distress (6%)
6> Cephalopelvic disproportion(3%)
7> pre eclampsia .
8> growith restricted foetus(3%)
9> obstructed labour (3%)
10> Others --> High risk pregnancies’ e.g Diabetes mellitus, Rh immunization, twin cord, prolapsed complication of caesarean section-->
A. Maternal complication-->
a. Immediate
1. Post partum Haemorrhage.
2. Shock.
3. Anesthesia hazards (by cardiac arrest)
4. Sepsis and wound infection.
5. Paralytic illus. is seldom
6. Leg venous thrombosis pulmonary embolism.
7. Pyrexia peritonitis, UTI, Burst abdomen, septic shock this is the commonest complication
b. Remote-->
1. Gynaecological—menorrhagia, (Menstrual excess of inner irregularities, cpp of backache)
amenorrhea, chronic pelvic pain, infertility vesico vaginal fistula.
2. General surgical complications- Incisional hernia and intestinal obstruction due to adhesions and bands.
3. Future pregnancy--> There is risk of scar rupture.
B. Fetal complications:-
1. If atrogenic prematurity and respiratory distress syndrome.
2. Injury to the baby due to surgical knife.
3. Birth asphyxia due to anesthetic problem.
